Discharge Summary workflow: When a patient is discharged from the hospital, a discharge summary is completed by the eligible provider. This document can be initiated by a transcriptionist. The eligible provider must however sign the final document before it is saved in the electronic medical record (EMR). When there is an error or an issue, the transcriptionist may escalate the issue to a supervisor or certified inpatient coder (CIC) who addresses the error and then forwards to the physician for signing.Arrange the steps below in the correct sequence and create a flowchart using the steps.Ensure to label the chart clearly.StartPhysician completes and electronically signs the documentTranscriptionist completes document and routes to attending physician’s inboxTranscriptionist starts the Discharge SummaryTranscriptionist forwards document to supervisor or CIC personnel via secure messagingPatient is discharged from the unit.Discharge Summary is automatically saved in EMR as final.Transcriptionist supervisor and/or CIC personnel makes any needed edits and routes to appropriate physician.EndDoes transcriptionist identify an error or issue that needs to be escalated?
