The health service plan will undertake health service review and development for one clinical stream for Sydney Local Health District, based on a choice of a series of options from two clinical position papers. These are:
• Sydney Local Health District (2012). Cardiovascular Clinical Stream Position Paper.
• Sydney Local Health District (2012). Aged Health, Chronic Care, Rehabilitation (ACC&R) and General Medicine Clinical Stream Position Paper.
Sydney Local Health District has made the Clinical Stream Position papers available on their web site and both of these papers include sufficient information to prepare a Healthcare Services Plan (HSP). However the clinical streams are very large and therefore students are to choose one of the specific topics for their plan:
• Cardiology and Cardiothoracic Surgery Services
• Renal Medicine Services
• Palliative Care Services
• Rehabilitation Services
• Acute and Sub-Acute Older Persons Services.
Students can review the Sunshine Coast HHS to see the separation of these services into specific clinical streams.
A health service plan needs to address services across the continuum of care including primary and secondary prevention, inpatient care, restorative care and management of ongoing health.
The data presented does not include inpatient activity projections apart from the analysis for future renal dialysis services, which could be included in the Renal Medicine HSP. Therefore this is not required in the HSP you develop apart from the data mentioned.

• Introduction
• Planning principles
• Demographic analysis including relevant population projections
• Needs assessment
• Service profile within Sydney LHD, including Role Delineation Levels and Models of Care
• Current activity levels, including separations and beddays by facility and across the LHD

• Situational analysis – Strategic Issues and Priorities
• Future Services Profile by Facility and across the LHD and Proposed Role Delineation Levels and Models of Care
• Key goals, objectives, strategies and recommendations in table format.

The health service plan will undertake health service review and development for one clinical stream for a de-identified Local Health District. Examples will be provided during Modules 4 and 5 and Workshop 2; but the selected topic could be an area such as women’s and children’s health services, renal services, emergency and critical care services, mental health services, cardio-vascular services, respiratory medicine services or surgical services planning. The specific topic will be notified to students together with relevant data sets and documentation for inclusion in the plan. A health service plan needs to address services across the continuum of care including primary and secondary prevention, inpatient care, restorative care and management of ongoing health.
The POE allows students working in the field to demonstrate application of POE measures. The facility examined may be one that provides hospital or community services, in the public, private or NGO sector. It may be a refurbishment or new project. It should be a unit (not a whole hospital) so that the operation of the unit is clear. It should not be more than 5 years old and should be at least one year old. Measures for use in the POE will be supplied in the relevant module.
