Your disease risk assessment report will:
give you the opportunity to assess your risk for communicable or non-communicable disease allow you to identify the factors increasing your risk for disease and what factors are controllable or uncontrollable allow you to develop a prevention strategy to reduce your risk for a disease
You may choose from one of the diseases listed below (may also propose a disease/condition not listed, but it must be approved by your professor) and complete the following:
Instructions:
1) Based on what you have learned throughout the course (review Units and corresponding textbook chapters on infectious and major chronic diseases), identify 1 disease or condition for which you feel you may be at greatest risk. Determine and reflect on your risk by interviewing family members about their health history, and/or assess the degree to which you engage in health-enhancing or health-compromising behaviours.
2) Your report should contain the following sections and headings (except for “f”):
a. Introduction (4 marks): Summarize the disease you have chosen. Provide only the most important background information on the disease. For example, explain what the disease is and discuss how the disease develops physiologically (e.g., how does it spread, how is it transmitted, how does it develop, how is it detected/diagnosed, why is it an important public health concern in Canada?). What age group or gender does the disease most commonly affect? Canadian population statistics? You will need to consult journal articles, your text, and/or reputable websites to obtain the necessary information for this section.
b. Controllable Disease Risk Factors (3 marks): Identify 2 major risk factors associated with the disease. Explain how each risk factor contributes to the onset of this disease in general. Then, describe how these two modifiable risk factors apply to you? *Note: None of us are impervious to disease/illness, so your selected risk factors should apply to you; do not select risk factors that are not relevant to you!!
c. Uncontrollable Disease Risk Factors (3 marks): Identify 2 uncontrollable risk factors associated with the disease. Explain how each risk factor contributes to the onset of this disease in general. Given your understanding of these factors and your health history, do any of these non-modifiable risk factors apply to you? Explain your answer.

d. Prevention Strategy (5 marks): Develop a 5 point prevention strategy for reducing your risk of the disease. How are you going to specifically going to reduce your risk? Consider modifiable lifestyle and health behaviours. Be specific. You may number your points 1. through 5, but please write in full sentences.
3) Citations and Conventions (2 marks): – You must reference your work and use at least 3 different reference sources. In addition you may use reputable health information sites (see below) and/or your textbook for information related to the disease you have chosen. You are free to use your text, online course Units, scientific journal articles, reputable health information websites (e.g., www.heartandstroke.ca), etc.
– Referencing (in-text and reference list) must be in APA format. See https://uregina.libguides.com/APA6th or use an APA referencing manual or use https://owl.english.purdue.edu/owl/resource/560/01/
– Format – please use headings as described above. This is not a traditional research paper so APA heading format is not required. A title page is needed – please use APA format for your title page.
– Spelling, grammar, flow, and punctuation will be included in this mark. Proof-read your report!!
4) Your report should be 1000 words minimum (not including reference list and title page). Try not to exceed 1200 words. Include only the most important and relevant information. Use headings listed above (yes, I’ve now mentioned this twice ). This is not a “traditional” research paper, but a report.
5) The report requires a title page. The report must follow 1 inch margins, 12 point font, double-spaced.
