For this Assignment, you will create a hypothetical client case study. You will fully describe the client’s demographics including age, gender, race, culture, religion, primary and secondary language spoken, city of residence, living arrangements, financial status, insurance coverage, employment history, social and familial support system, lifestyle factors, mental health history, personal interests and any other demographic information that you believe should be addressed about your client including (but not limited to) sexual orientation, relationship status, family dynamics issues, or legal issues. When creating your case, create a
picture of your hypothetical client’s life as you would want if you were going to be working with this client and were seeking to gain an understanding of their unique circumstances.
You work for an agency that provides case management for low income seniors who live at home and for whom English is a second language. Your client has recently lost a spouse after 60 years of marriage. Create a scenario where your hypothetical client comes to you seeking information about coping with grief and loss as well as resources to seek grief counseling and financial support. Craft a client care plan that includes:
• An introduction to your client, their demographics and life circumstances.
• Your approach to assessing all cultural factors, including consideration of verbal and nonverbal responses, that will impact a client who speaks English as a second language, and the inclusion of the specific methods utilized to ensure your client is able to understand and fully participate in the assessment and care plan with consideration of their native language, culture, ethnicity,
and/or religion including at least one citation from a professional source to support your method (s).
• Your recommendations and interventions (such as evaluating for eligibility for Medicaid and VA benefits, completing a depression screening) including at least one citation from your text or professional source (such as published research,
professional journals or our text) to support your recommendations.
• A resource attainment plan including contact information for the resources to be provided and how they will be accessed by the client (please cite all included resources within your care plan and include the citations on your reference page)
• Exploration of client strengths that will aid in plan of care and how those strengths may be effectively utilized (such as client having Medicare coverage to pay for grief counseling).
• Exploration of potential barriers that will create challenges in plan of care and how they may be navigated (such as lack of family support).
• Discussion of any impacts of culture or religion on recommendations made and the client’s verbal and nonverbal responses to those recommendations.
• A conclusion (please include an outcome for the client demonstrating success or continued challenge for reaching the client’s stated goal of enhanced emotional and financial support)
Your Assignment should include a cover page, introductory paragraph, citations, concluding paragraph, and reference page. It also should be free of spelling and grammatical errors. The body of the work should be 35 doubles-paced
pages (a total of 57
pages including the cover page and reference page).
